To demonstrate support of the hospital mission and vision by providing nursing, case management/disease management services to clients and families to help in the coordination and management of individuals’ health needs through the full continuum. The Registered Nurse demonstrates the ability to assess, coordinate, plan, monitor, implement and evaluate the patient’s plan of care while ensuring and facilitating the achievement of optimal quality, clinical and financial outcomes.  Performs assessment of clinical and life issues for the patients of COH. Provides professional nursing services in support of the physician, in the direct and indirect care of patients.

The successful candidate:

  • Must be able to work Monday-Friday, 8:00a-4:30p.

  • Monitor and address oncology patient symptoms, including chemotherapy side effects.

  • Educate patients and families about disease management and treatment plans.

  • Document assessments, toxicity screenings, and interventions in the electronic medical record.

  • Collaborate with healthcare providers to assist in coordinating patient care.

  • Evaluate patient health concerns and communicate findings to physicians.

  • Effectively triage patient medical issues in person or via telephone.

  • Provide professional nursing services, working alongside clinical and clerical teams.

  • Facilitate communication between healthcare teams, patients, families, and external agencies.

  • Ensure positive patient experiences through excellent customer service and prompt issue resolution.

  • Respond to patient calls and walk-in appointments in alignment with hospital turnaround time goals.

Qualifications

Your qualifications should include:

  • Current Georgia RN license from an accredited college

  • BSN required or enrolled within 6 months of hire and BSN completed within three years from hire date

  • Current BLS

  • Current ACLS preferred

  • OCN Certification preferred

  • 1 year clinical oncology experience preferred
