The successful candidate:
Hours: Monday - Friday, 8:00 AM - 4:30 PM, with coverage every 8th weekend
Job Description: We are seeking a dedicated and knowledgeable Clinical Oncology Pharmacist to join our team. The specialist will provide evidence-based, patient-centered pharmacotherapy management for hematology and stem cell transplant patients. As the subject matter expert for the multidisciplinary care team, you will assess chemotherapy regimens and supportive care guidelines, offer recommendations to physicians and advanced practitioners, and provide medication education to patients and healthcare team members. You will also serve as the pharmacy representative for the stem cell transplant unit and coordinate with pharmacy technicians to ensure safe and timely medication delivery.
Key Responsibilities:
Provide evidence-based, patient-centered pharmacotherapy management for hematology and stem cell transplant patients.
Assess chemotherapy regimens and supportive care guidelines.
Offer recommendations to physicians and advanced practitioners.
Provide medication education to patients, physicians, and other healthcare team members.
Serve as the pharmacy representative for the stem cell transplant unit.
Coordinate with pharmacy technicians to ensure safe and timely medication delivery.
Minimum Qualifications:
Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education.
Current Illinois State Registered Pharmacist license.
Minimum of 3 years of clinical pharmacy experience, preferably in oncology.
Completion of a 1–2-year residency program.
Board Certification in Oncology Pharmacy (BCOP) to be acquired within 2 years of employment.
